Bacterial tracheitis: a therapeutic approach.

چکیده
OBJECTIVES/HYPOTHESIS tracheitis is an acute, infectious, potentially life-threatening condition of the pediatric airway. Historically, patients have often required urgent invasive airway support, and have been treated with broad-spectrum antibiotics, often combined with direct laryngoscopy and bronchoscopy. STUDY DESIGN Retrospective chart review. METHODS Six patients between the ages of 10 months and 16 years were treated at Children's Hospital, Boston, Massachusetts, for bacterial tracheitis between January 2009 and March 2009. All patients underwent urgent direct laryngoscopy and bronchoscopy for debridement of mucopurulent debris and tissue culture. Broad-spectrum intravenous antibiotics were administered, and patients were kept on acute cardiopulmonary monitoring for 48 to 72 hours, after which time the airway was re-evaluated by direct laryngoscopy and bronchoscopy in five of the patients, and by fiberoptic nasolaryngoscopy in one patient. All of the patients were transitioned to oral antibiotic therapy for 10 to 14 days after discharge. RESULTS Following debridement, all of the patients did well with aggressive medical management. None of the patients required urgent intubation, although one patient was kept on ventilator support for 48 hours until disease resolution was confirmed. No patients required tracheotomy, and there were no cardiopulmonary arrests. The mean hospital length of stay was 4.8 days (range, 3-8 days). CONCLUSIONS This study highlights the importance of early disease identification and urgent surgical intervention in the management of bacterial tracheitis. It also demonstrates an approach with the potential to improve patient outcomes from this dangerous condition.

Systemic complications associated with bacterial tracheitis.
The toxic shock syndrome, septic shock, pulmonary oedema, and the acute respiratory distress syndrome (ARDS) were recognised in four children with bacterial tracheitis. ARDS has not previously been reported in association with bacterial tracheitis. Prompt recognition of the severe systemic complications of bacterial tracheitis could lead to a decrease in the morbidity and mortality of this cond...
متن کاملBacterial Tracheitis: A case report
Bacterial tracheitis is an infectious cause of acute upper airway obstruction in children. Although tracheitis presents similar to croup with respiratory distress and stridor, this disease entity must be differentiated from the latter early, as the therapy for each disease is different. We present a case of bacterial tracheitis describing the clinical course, pertinent laboratory and radiologic...
متن کاملExtracorporeal membrane oxygenation for refractory air leak in a child presenting with bacterial tracheitis.
In developed countries, bacterial tracheitis is an uncommon cause of acute upper airway obstruction in children, but the majority (57–91%) of patients who present with this illness require invasive mechanical ventilation.1-3 Bacterial tracheitis is also associated with numerous complications, including ARDS, air leak syndromes, septic shock, and multiple organ dysfunction.1-3 We describe the fi...
متن کاملBacterial tracheitis in Down's syndrome.
Four children with Down's syndrome and bacterial tracheitis are described. In three the infection was due to Haemophilus influenza. In patients with Down's syndrome presenting with stridor tracheitis should be considered and appropriate treatment started.
متن کاملChanging epidemiology of life-threatening upper airway infections: the reemergence of bacterial tracheitis.
OBJECTIVE As a consequence of evolving medical practice, the epidemiology of potentially life-threatening upper airway infections is changing. We report our experience over 9 years with viral croup, epiglottitis, and bacterial tracheitis. PATIENTS AND METHODS We studied a retrospective case series of patients admitted to Vermont Children's Hospital with potentially life-threatening upper airw...
